How do I fill in auto-entrepreneur form 2042 C?

I’m an auto-entrepreneur and I declare and pay my taxes online. I registered on 1 October 2014 and I’ve paid the first taxes on 31 January. I received an attestation to say that I declared x amount and now I have to fill in a 2042 C form.

Where can I find the form for 2014?
How do I fill this form in?
Do I have to do it every trimester?
