Application for agrement organisme formation DIF

I’m an auto-entrepreneur and just about to make an application for an agrement organisme formation for the DIF and on the website Service Public. They ask for the following document:

justificatif d’inscription sur la liste arrêtée par l’organisme collecteur paritaire agréé (OPCA), dans le cas où l’organisme présente à l’appui de la déclaration une convention de bilan de compétences pour un salarié.

Could you please explain what this is and how I can get it?
