After registering a business in France when do I get my carte vitale?

I registered an auto entrepreneur business in France in August 2015. I have yet to receive my carte vitale. I have a paper that says I have health coverage and I have a form to fill in with the details of my doctor with - but as I haven’t had the need for a doctor so far I haven’t filled in the form. Do I get my carte vitale only after I return this form?
