AE statement of non-conviction?

I am being asked to provide:
A sworn statement of non-conviction showing the parentage of X
(The declaration must show the filiation, that is to say the name and the first name of your father as well as the maiden name and the first name of your mother.)
Is this something I can just write out and sign? Or what do I need for this?
