Add family members to carte vitale?
I am trying to complete the form Declaration en vue du rattachement des membres de la famille des assures pour le benefice de l’assurance maladie et maternite. I thought I had understood the supporting documents needed for the children. I have four 18, 17, 10 and 9 yrs old, but I need help?
The youngest two are in the local school and RAM are asking for: Photocopie de la carte nationale d’identité ou du passeport en cours de validité et photocopie du livret de famille. Please can you expalin this?
My 17 year old is currently not at school but waiting to see if he can start an Apprenticeship part work/part lycee this September and my eldest son 18 yrs. is registered with an American high school and does his studies on-line although he sits his exams at a Lycee in Narbonne (we lived in the USA for the last 4 yrs). All children live at home.
For the older two children they are requesting I submit: Photocopie de la carte nationale d’identité ou du passeport en cours de validité, photocopie du livret de famille et attestation de scolarité en cours. Please can you explain this? How do I satisfy RAM with these documents?
