How to fill in section C of the cotisation fonciere form?

I am filling in my first Cotistation Fonciere form for 2016. I registered as an auto-entrepreneur business in France in April of this year. I am following your detailed notes which are great. However, I have a question regarding section C. I am an English teacher and I teach in my client’s’ offices in central Paris. But I prepare all my lessons at home outside of Paris and may in the future have lessons at my house.

Does that mean in Section C #1, I should put maison as it is my home and then give my home address as the address in #2?
For questions #8, #14 and #21 what should I input? I only use our dining room table as my office. I saw in another post that someone else was in this situation and you suggested that they put down around 4m2? Can you confirm this?
Again for #21, I do go to other locations for work - should I fill this box in?
Finally, for #11, if I give 4m2 as the amount of space utilized for professional work, am I right to say then the rest of the house (in this case 140m2 - 4m2 = 136m2) is habitation?
