How does a profession liberale micro-entrepreneur add a second NAF or APE?

You answered this question in 2014 but I’m not sure if your answer applies to all types of micro-entrepreneur businesses in France. You indicated it wasn’t possible to add secondary NAF/APE codes online for a profession liberale micro-entrepreneur.

Where would I need to visit in person?
If I wanted to do this by post, where would I find the form I need to use and where would I send it?
