How do I fill in cotisation fonciere des enterprises 2015 form?

I received a document last week Cotisation fonciere des enterprises 2015. I registered as chambres d hotes and sejours linguistiques on 1 April this year. I am not sure how to fill in the form.

A2 origine d establishment - debut d activite?
A3 identification de l ancient exploitation ?
B1 date de creation 1 April?
Chiffres d affaires income from April to?

There is lots on this form I don’t understand especially the adjustment des affaires de l’annee nature de bien - chambres d hotes or maison m2 professionel Industrial Commercial? As a chambres d’hotes the only rooms exclusively for guests are the bed and breakfast rooms all other areas are for guests and family use, so is it habitation - our living space and commercial bed and breakfast rooms do we include bathrooms in the M2?
