Where do I send my demande d’ACCRE?

I just did my auto-entrepreneur declaration online this morning. I qualify for l’ACCRE, so I selected the appropriate box then printed and filled out the form. Now where do I send it? I received an email confirming my télédeclaration from .(JavaScript must be enabled to view this email address) and this e-mail says that my declaration is being processed at this address:

Should I send it there? With a letter citing my numéro du dossier? With a PDF copy of my declaration? With a copy of the confirmation email?
