Should I complete CMU forms for Base Protection?

I registered as an auto-entrepreneur in April 2011 and have just received a long form from the CPAM entitled Couverture Maladie Universelle, Protection de Base. I have filled one of these in before but now I’m affiliated with the RSI I didn’t think I had to fill this in again.

I have always paid a top up insurance and have never got this from the CPAM. It’s a long and tedious form and they want tout autre justificatif de ressources du 1/1/10 to 31/12/10 - I’m sure I’ve already sent one of these back in 2010. I am really confused about this and any help would be gratefully received.
