S1 holder to become auto entrepreneur. What happens to family health cover?

My family is covered under my S1, but what happens to their health cover once I have my auto entrepreneur Carte Vitale. Are they attached to my Carte Vitale and therefore on the French Social Security system, or do we have to pay for a mutuelle for a 100% health cover?
