Paper or online cotisation foncière des entreprises (CFE) form?

My husband and I are both auto-entrepreneurs. Following instructions on emails that we received, I registered us both to pay our professional tax cotisation foncière des entreprises (CFE) online. My husband received an activation code and mine has not arrived. Then I received a paper form to fill in for CFE and my husband hasn’t had one.
My business commenced in April 2014 and his in October 2014. Does my husband need to complete a form too, and if so, how would he get one?
