How to obtain my Carte Vitale?

I am a Professional Liberale, our SARL was started July 2010, and I duly received a letter Nov 16th of Attestation D’Affiliation. However this letter only referred to me personally [EU citizen] (Gerant) but not my wife, a US citizen with 10 yr Carte de Sejour, who is listed within the company as a non salaried Associate. The birth certificates for both of us etc. were sent already to RSI.

How to obtain the Carte Vitale(s) from here?
How to ensure cover for my wife?
Can you recommend any top up policy providers and selection tips?
