How do I set up a primary and secondary business here in France?

My wife is an artist and will register as an Artiste Libre with the Centre Finances Publiques. We understand that she does not need to pay tax or social charges up to 8,000 euros income. Her second activity will be renovating brocante and crafts. We understand that this is activite artianals and normally handled by Chambres de Metiers. Her art income will be 6,000 euros and the crafts income will be 3,000 euros

Can my wife apply to Centre Finances Publiques for both activities at the same time? Also, as the crafts is secondary does she have to do the five day course?
We really just want to simply fill out one form to register both businesses at the same time in one place. Is this possible? What would be the cost?
She will be an auto entrepreneur. Would she be able to get her own Carte Vitale?
