How do I fill in the declaration trimestrielle form?

I have been re-registered and it has been back-dated to January last year. So now URSSAF has sent me the Declaration Trimestrielle for all of 2013 and 2014 to date. I would like confirmation as to how I fill out the form. For example for making a declaration on €5000 I presume that I enter the 5000 under the heading Recettes Trimestrielles Arrondies and on the line Activites Relevant de la CIPAV. But what do I put on the next line starting with Code 578?
