How do I get my carte vitale documents translated?

I am an auto-entrepreneur since last February but have not yet received my Carte Vitale. I sent the RSI various documents as requested. The latest response asked me for a translation of our (my wife and I) birth certificates. Can I translate these myself or must they be professionally translated? How do I go about finding a professional translator? I live in the Montauban area. Also they are looking for an Attestation de fin de droits ancient regime and an Attestation CERFA. What are these?
