Creating a personal account with RAM to declare doctor

Hi Valérie, Hi Forum members,

I have received a letter from RSI/la Ram, Bienvenue à la RAM. In this letter, they indicate that I need créer votre compte en ligne personnalisé...” and to enter my RIB pour le versement rapide de vos remboursements….

Also they have included a form with the letter, the Déclaration de médecin traitant form, although there is no mention of the form in their letter.

I have a couple of questions:

Do I have to complete the déclaration de médecin traitant form again? I have already done this in the past for the CPAM and already have a Carte Vitale - however, in the original declaration, I was the bénéficiaire under my husband.
Is it usual practice to create a personal account on RAM?

Just wanted to check these two points with you before I go ahead.
