Child allowance for auto-entrepreneurs?

I have been living in France for 5 years on a expat contract. Till end 2011 we received child allowance from our home country (NL). We stopped this because since 01.01.2012 I am registered as an auto-entrepreneur. We would like to apply for child allowance at CAF but got stuck in all the different papers. It seems that we need to have a number allocataire. How can we get this number? And as an auto-entrepreneur can we just use the regular application form for child allowance or is there a different one?
