Birth certificate format and translation to set up a business in France?

I have been asked by régime social des indépendants (RSI) to send a copy of my birth certificate with the names of my mother and father on it. I do not possess one as I was adopted at the age of five and only have a birth certificate dated from then - with my adoptive name on it. How do I get around this?

Also it asks for a sworn translation either by any one of the following:

a sworn translator from a court of appeal in France
the embassy or consulate of the country of origin in France
the embassy or the French consulate in the country of origin

How do I go about this?
