Attestation Fiscale 2016 - finding a copy?

I usually receive a paper copy of the French Attestation Fiscale but this year I haven’t received one. I looked up a previous answer to a similar question on here and saw that I should be able to download one from Net Enterprises.

I now have an account on Net Enterprises but I can’t seem to find it on the site under my French account. I also clicked through to the URSSAF site but I can’t seem to find it there either.

I have the montant des recettes from my payments in 2016. However, I don’t know which ‘cases’ to put them on my joint tax declaration with my husband that we need to complete by 6 June. They are different to 2015. In 2015 I had to put them in cases 5HQ, 5IQ et 5JQ.
