Which forms are sent out for quarterly micro entrepreneur turnover declaration?

I set up as a micro entrepreneur in Frabce last year and each quarter in 2017 I received the paper declaration form. 

It is now April and I have not received the paper form.  Must I complete on line? I prefer to do the paper submission.
